Dermatitis Herpetiformis When Gluten-Free Diet Fails to Control Skin Disease

For patients with dermatitis herpetiformis (DH), a strict and lifelong gluten-free diet (GFD) is the established first-line intervention. When this dietary approach does not achieve adequate clinical improvement of the skin manifestation, a defined next-line protocol applies.

Prior treatment — insufficient response

A strict and lifelong gluten-free diet is the initial treatment for all DH patients. When the expected goal — clinical improvement and response of the skin manifestation — is not reached despite dietary adherence, escalation is indicated. This protocol addresses that situation.

Next-line approach

When the gluten-free diet alone is insufficient, adding dapsone to the ongoing dietary regimen is the indicated next step.
