Treatment of Mild Dengue Fever When Oral Fluids Are Tolerated and No Warning Signs Are Present
Patients presenting with mild dengue who can take adequate oral fluids, are voiding urine regularly, and have no dengue warning signs represent a specific group managed through a defined outpatient approach centred on hydration, rest, and temperature control.
Clinical scenario — all criteria must be met
- Mild dengue fever
- Able to tolerate adequate volumes of oral fluids
- Able to pass urine at least once every 6 hours
- No dengue warning signs present (particularly important as fever subsides)
Management approach — partial overview
Management rests on adequate bed rest and maintaining oral fluid intake using appropriate fluid choices, with specific fluids recommended and carbonated drinks to be avoided. Fever is addressed with a specific antipyretic once body temperature exceeds a defined threshold, while certain analgesics commonly used for fever must be avoided. Physical cooling of targeted body areas is also part of the approach.

- Able to tolerate adequate volumes of oral fluids.
- Able to pass urine at least once every 6 h.
- Not having any of the warning signs (particularly when fever subsides).
- Patient needs to take adequate bed rest.
- Adequate intake of fluid such as milk, fruit juice, isotonic electrolyte solution, oral rehydration solution (ORS) and barley/rice water.
- Body temperature should be kept below 39 °C. If the temperature goes beyond 39 °C, give the patient paracetamol. Paracetamol is available in tablet form or in syrup form. The recommended dose is 10 mg/kg/dose and should be administered in frequencies of not less than 6 h. The maximum dose for adults is 4 g/day. Avoid using too much paracetamol, and aspirin or NSAID.
- Tepid sponging of forehead, armpits, and extremities. A lukewarm shower or bath is recommended for adults.
- Prescribe only paracetamol for fever; do not give NSAIDs as it may cause bleeding.
