This protocol addresses patients with Darier disease presenting with localized skin lesions (not disseminated or generalized) who have not achieved the expected therapeutic outcome with a prior course of topical retinoids.
A first-line course of topical retinoids was used with the treatment target of complete resolution of symptoms and marked improvement of the skin lesions. When those goals are not reached, escalation to the next treatment step is indicated.
For severe, resistant localized lesions that have not responded adequately to retinoids, topical fluorouracil (5-FU) has been used in reported cases, with responses observed within weeks of treatment initiation. The complete protocol specifies concentrations, application details, and follow-up criteria.
The target outcome is marked improvement and resolution of the localized lesions.

A total of 11 patients (eight females, three males) have been reported to use topical 5‑FU at dosages of 1%, 5% or 20% for severe and resistant‑to‑therapy Darier disease.
All patients showed marked improvement and resolution after 2 weeks of treatment but relapsed after treatment cessation.
