Treatment Approach

Treatment Goal

The primary clinical endpoint is disappearance of the retinal necrotic lesion on fundus photograph within two weeks of initiating anti-cytomegalovirus therapy.

Initial treatment typically involves first-line drugs like ganciclovir (GCV), valganciclovir (VGCV), cidofovir (CDV), and foscarnet (FOS).
GCV is the first-line treatment for CMV, offering various administration routes including oral, intravenous, vitreous, and intraocular implants.
Research has shown that a twice-daily dose of 900 mg of oral VGCV for induction therapy in CMVR patients has an efficacy and safety profile comparable to that of intravenous ganciclovir.
After 2 weeks of anti-cytomegalovirus therapy, retinal necrotic lesion has disappeared on fundus photograph, OCT showed subretinal fluid absorption and the edema of the necrotic lesion relief.
