This protocol addresses patients with cutaneous lichen amyloidosis in whom initial treatment with topical DMSO has failed to achieve adequate symptom control. When the targets of that first-line regimen are not met, a structured next-line approach is indicated.
Topical DMSO (applied daily, with intermittent maintenance once pruritus was partially reduced) was the prior regimen. The goals it was expected to meet — significant reduction of pruritus, pigmentation, and papules — were not achieved, triggering escalation to this protocol.
The next step centres on a phototherapy-based strategy. More than one modality within this category may be applicable — the specific selection, sequencing, and clinical algorithm are set out in the full protocol.
