Treatment of Cutaneous Leishmaniasis Caused by Leishmania guyanensis

This protocol applies to cutaneous leishmaniasis where the causative species is Leishmania guyanensis. Species identification in this setting is clinically relevant because the causative organism directly determines which treatment approach is appropriate.

Systemic treatment is required for L. guyanensis — local treatment is not recommended in this setting. A specific systemic agent is used as first-line therapy, with an oral alternative. The complete regimen, including drug selection and all relevant parameters, is available in the full protocol.

Treatment of L. guyanensis

All other cases—systemic treatment:

Pentamidine is the first line treatment for L. guyanensis CL in French Guyana, Surinam, and Brazil, with cure rates around 90%.

Treatment failure is present when reepithelialization is incomplete 3 months after starting therapy.
