Treatment of Cutaneous Amoebiasis in Severe Disease or When Oral Therapy Is Not Tolerated

Cutaneous amoebiasis requires prompt treatment selection based on disease severity and the patient's ability to receive and absorb oral medication. When standard oral therapy is not feasible, a specific intravenous approach applies.

Clinical Scenario

This protocol addresses cutaneous amoebiasis in the setting of severe disease or in patients who are unable to tolerate or absorb oral therapy. In these situations, an alternative route of administration is required to achieve adequate drug exposure and clinical response.

Treatment Approach — Partial Overview

Management involves an intravenous nitroimidazole agent, used because oral therapy is not viable in this setting. Because this drug class does not reliably eradicate luminal cysts, it must be followed by a luminal cysticidal agent given by mouth once oral intake is possible.

In patients who are unable to tolerate or absorb oral metronidazole, intravenous metronidazole should be used.

Severe disease or unable to tolerate oral therapy, use metronidazole 1500 mg IV divided TID (7.5–30 mg/kg/d divided TID).

The nitroimidazoles do not effectively eradicate luminal cysts and must be followed by a luminal agent.
