Treatment of COVID-19 Pneumonia in Hospitalized Patients with Mild-to-Moderate Disease at High Risk for Progression to Severe Illness
This protocol applies to the hospitalized patient with mild-to-moderate COVID-19 — defined as SpO2 greater than 94% on room air, not requiring supplemental oxygen — who is at high risk for progression to severe disease and presents within 7 days of symptom onset.
The defining feature of this population is preserved oxygenation without supplemental support, combined with recognised risk factors that increase the likelihood of clinical deterioration. Early intervention within the established symptom window is central to the recommended approach.

Among patients (ambulatory or hospitalized) with mild-to-moderate COVID-19 at high risk for progression to severe disease, the IDSA guideline panel suggests remdesivir initiated within 7 days of symptom onset rather than no remdesivir.
Mild-to-moderate illness is defined as patient with SpO2 >94% not requiring supplemental oxygen.
