Treatment of High-Risk Gestational Trophoblastic Neoplasia (FIGO Score ≥7) Following Complete Molar Pregnancy

Clinical Scenario

This protocol applies to women with gestational trophoblastic neoplasia (GTN) arising after a complete molar pregnancy whose pre-treatment FIGO 2000 score is 7 or greater, placing them in the high-risk category requiring intensive systemic treatment.

Risk Classification
High Risk — FIGO 2000 Score ≥ 7

Women are assessed before chemotherapy using the FIGO 2000 scoring system. Those with a score of 7 or greater are classified as high risk. This scoring drives the choice of treatment intensity and distinguishes this population from lower-risk GTN.

Treatment Approach & Goals

High-risk GTN in this setting is managed with intravenous multi-agent chemotherapy. Treatment is directed towards normalisation of hCG levels, and continues beyond that point for a further defined period.
