When Cogan's syndrome presents with cochleo-vestibular involvement at moderate to severe intensity, the clinical picture is particularly serious. This scenario carries a poor prognosis for auditory and vestibular function and demands rapid initiation of appropriate therapy.
Clinical scenario: Moderate to severe cochleo-vestibular involvement in Cogan's syndrome. Unlike ocular manifestations of the disease, cochleo-vestibular involvement has a poor prognosis and justifies the rapid introduction of immunosuppressive treatment.
First-line management combines systemic corticosteroid therapy with an immunosuppressant agent. An adjunctive local intervention targeting the inner ear may also be considered alongside systemic treatment.
