Treatment of Coats' Disease at Shields Stage 1 or Stage 2 with Disease Progression

Coats' disease at Shields stage 1 (retinal telangiectasia only) or stage 2 (telangiectasia with exudation), when active disease progression is documented, requires prompt, targeted intervention to prevent advancement to more severe stages.

This protocol addresses early-stage Coats' disease with demonstrated progression. At stage 1, the retina shows telangiectatic vessel changes without exudation. At stage 2, exudation is present alongside the telangiectasia. Both stages remain amenable to intervention directed at the abnormal vasculature before vision-threatening complications arise.

The primary treatment targets the abnormal retinal vasculature directly. Laser photocoagulation is the established first-line approach at these early stages — the specific technique, retreatment strategy, and how many sessions may be needed are detailed in the full protocol.

Clinical goal: Ablation and regression of the abnormal retinal vasculature, with preservation of vision and prevention of disease progression.
