Treatment of C. difficile Infection in Inflammatory Bowel Disease with Acute Flare

In patients with inflammatory bowel disease (IBD), an acute flare presenting with diarrhea and confirmed Clostridioides difficile infection (CDI) represents a specific clinical scenario that requires a defined treatment approach.

Clinical scenario: Inflammatory bowel disease with an acute flare associated with diarrhea and confirmed Clostridioides difficile infection. C. difficile testing is recommended in IBD patients presenting with this combination.

Treatment approach

Management in this setting is guided by an oral antibiotic regimen selected specifically for IBD patients with confirmed CDI. The complete dosing, duration, and clinical decision pathway are available in the full structured protocol.

We recommend C. difficile testing in patients with inflammatory bowel disease (IBD) presenting with an acute flare associated with diarrhea (strong recommendation, low quality of evidence).

We suggest vancomycin 125 mg p.o. 4 times a day for a minimum of 14 d in patients with IBD and CDI (strong recommendation, very low quality of evidence).
