Not all chronic rhinosinusitis presentations follow standard management. Severe symptoms or the presence of specific red-flag signs indicate a clinical situation that requires a distinct, urgent approach.
This protocol applies when chronic rhinosinusitis presents with any of the following:
Red-flag signs or symptoms should elicit urgent evaluation to rule out more serious pathology. Unilateral symptoms warrant expedited assessment, as chronic rhinosinusitis is typically bilateral in nature.
When red-flag signs or severe symptoms are present, the management pathway diverges significantly from standard chronic rhinosinusitis care.

Unilateral symptoms should prompt an expedited referral to rule out other pathologies as chronic rhinosinusitis is typically bilateral in nature.
Urgent referral to otolaryngologist and urgent CT of the paranasal sinuses.
