Structured Endoscopic Treatment Approach for Chronic Recurrent Parotitis
Chronic recurrent parotitis presents with repeated episodes of parotid gland inflammation and swelling. When the clinical picture calls for a procedural intervention, a structured protocol targeting the ductal anatomy of the affected gland provides a systematic path forward.
This first-line protocol applies to patients with chronic recurrent parotitis in whom endoscopic evaluation and ductal intervention of the affected parotid gland is indicated.
Treatment approach — partial overview
The protocol centres on sialoendoscopy of the affected gland, employing an endoscopic technique that accesses the parotid ductal system through its natural orifice. The complete procedural sequence, instrumentation steps, and decision points are available in the full protocol.

- The first step is to dilate the orifice of the Stensen's duct with lacrimal probes up to 1.3 mm (the outer diameter of the Sialoendoscope diagnostic unit [Karl Storz GmbH, Tuttlingen, Germany]).
- After the dilatation, we introduce the diagnostic unit intraductally.
- The advancement is done under continuous lavage with isotonic saline solution.
- The first attempt to dilate the stricture is done with saline pressure via introduction of the diagnostic unit forward and simultaneous injection of saline through the irrigation port.
