Treatment of Chronic Myringitis with Recurrent Acute Otitis Media and Middle Ear Effusion

This protocol addresses children presenting with recurrent acute otitis media — defined as three or more episodes within six months, or four or more episodes within 12 months with at least one episode during the preceding six months — accompanied by middle ear effusion.

In this setting, a surgical intervention targeting the middle ear may be considered as an alternative to observation, with specific guidance on how subsequent ear drainage should be managed. The full evidence-based regimen — including indication criteria, procedural considerations, and management of complications — is available in the complete protocol.

For children with recurrent AOM (i.e., three or more episodes in six months, or four episodes within 12 months with at least one episode during the preceding six months) with middle ear effusion, tympanostomy tubes may be considered to reduce the need for systemic antibiotics in favor of observation, or topical antibiotics for tube otorrhea.

Children with tympanostomy tubes who present with acute uncomplicated otorrhea should be treated with topical antibiotics and not oral antibiotics.
