Patients whose CLL progresses after a fixed-duration venetoclax-containing regimen should be offered either a covalent BTKi (GRADE III) or a venetoclax-based regimen depending on prior response, tolerance and patient preference (GRADE 1B).
For patients relapsing within 3 years of fixed-duration therapy, switching to a BTK inhibitor is generally preferred.
For later relapse (>3 years), retreatment with venetoclax-based therapy is reasonable, and all options may be considered.
