Treatment of CLL With Disease Progression on a Covalent BTK Inhibitor

Patients with chronic lymphocytic leukaemia (CLL) who have experienced disease progression while on a covalent BTK inhibitor represent a defined clinical scenario with specific treatment considerations.

This protocol applies to patients with chronic lymphocytic leukaemia who have a prior covalent BTK inhibitor in their treatment history and whose disease has progressed on that covalent BTK inhibitor.

Idelalisib in combination with rituximab is among the regimens considered in this clinical context.

Full regimen details, selection criteria, and evidence grading are available in the complete protocol.

  1. Patients whose CLL progresses on a covalent BTKi should be offered either a venetoclax-based regimen, irrespective of TP53 status (GRADE 1B) or Pirtobrutinib (Grade 1B, EMA, MHRA approved).
  2. Idelalisib–rituximab remains an option for patients who are refractory to BTKi-and BCL2i-based treatment, or for whom these treatments are unsuitable (GRADE 2B).
