Treatment of Grade 2–3, Clear Cell, or Pelvic/Axial Skeleton Chondrosarcoma

This protocol covers patients with higher-grade chondrosarcoma — specifically grade 2 or grade 3 disease, clear cell chondrosarcoma, or chondrosarcoma arising in the pelvis or axial skeleton. Dedifferentiated, mesenchymal, and skull-base subtypes are outside this pathway.

Higher-grade chondrosarcomas (grade 2–3) and clear cell chondrosarcoma, as well as chondrosarcomas located in the pelvis or axial skeleton, constitute a distinct clinical sub-population where the extent and quality of surgical resection are a primary consideration. This is a first-line protocol for these presentations.

Higher-grade chondrosarcomas (including clear cell chondrosarcoma) and all chondrosarcomas of the pelvis or axial skeleton should be surgically removed with wide margins.

Local recurrence of chondrosarcoma is best treated by further wide excision.
