Treatment of Cholesterol Atheroembolism with Concurrent Atrial Fibrillation

Cholesterol embolization syndrome presents a specific management challenge when the patient also has a compelling, concurrent indication for anticoagulation. This scenario has direct clinical guidance.

This protocol addresses cholesterol embolization syndrome (cholesterol atheroembolism) in patients who carry a concurrent mandatory indication for anticoagulation therapy — specifically atrial fibrillation, a mechanical prosthetic heart valve, or a new intravascular thrombus. Current consensus supports continuing anticoagulation in these patients despite the diagnosis of cholesterol embolization syndrome.

The approach combines continued anticoagulation with measures to suppress the inflammatory response to cholesterol crystals, alongside supportive care. The full regimen — including specific agents, sequencing, and monitoring — is detailed in the complete protocol.

The primary treatment target is improvement in renal function.
