Cholera in Children Under 18: What to Do When Oral Rehydration and Zinc Have Not Worked

Clinical Scenario

This protocol is for patients under 18 years of age with cholera and without severe acute malnutrition. The clinical targets are resolution of dehydration and cessation of watery stools.

Previous Treatment — Escalation Trigger

The first-line approach used oral rehydration with ORS (Oral Rehydration Solution) and zinc supplementation. This next-step protocol is indicated when the child has not met all of the following targets on that initial treatment:

Next-Step Approach (Partial Overview)

When oral rehydration has been insufficient, this protocol centres on intravenous fluid resuscitation, transitioning to oral therapy as soon as the child can tolerate it. A single-dose oral antibiotic is also part of the regimen. The specific agents, sequencing, and full clinical algorithm are in the complete structured protocol below.

Goals: resolution of dehydration — cessation of watery stools.
