Treatment of Choledocholithiasis with Common Bile Duct Stones Larger Than 1.5 cm or Difficult Stone Anatomy

Not all common bile duct stones can be removed with standard sphincterotomy and basket or balloon extraction. When specific stone or anatomical features make extraction difficult or predict failure, a structured approach is needed.

Clinical Scenario — Difficult Stone Criteria

This protocol applies when one or more of the following are present:

Stone diameter over 1.5 cm
Barrel-shaped stone
Sigmoid common bile duct
Stone located in the cystic or intrahepatic ducts
Stone impaction
Multiple common bile duct stones
Narrow distal common bile duct
Angled common bile duct

The criteria for difficult gallstones are as follows: diameter over 1.5 cm, barrel-shaped, sigmoid or located in the cystic and intrahepatic ducts, and stone impaction, as shown in Figure 8.
Predicted failed extraction by sphincterotomy + balloon and/or basket (stone size >1.5 cm, multiple stones, narrow distal CBD, angled CBD)
In these difficult gallstone cases, dilation of the bile duct uses endoscopic papillary large balloon dilation (EPLBD).
This procedure is done after endoscopic sphincterotomy.
