Treatment of Chilblains When Preventive Measures Are Inadequate

Chilblains (perniosis) are localised inflammatory lesions provoked by cold-damp exposure. Protective and behavioural measures are the cornerstone of management, but a subset of patients does not achieve adequate control with prevention alone.

When cold-avoidance and other preventive strategies have proved insufficient to control active lesions, the clinical situation calls for a structured pharmacologic step. This protocol addresses exactly that scenario.

Treatment goal

Resolution of chilblains lesions within 7–10 days.

When preventative measures alone are inadequate, oral nifedipine is generally recommended as first-line pharmacologic therapy.

Dowd et al. demonstrated that 7/10 patients treated with nifedipine experienced resolution of lesions within 7-10 days.
