Chalazion with Suspected Viral Etiology, Masquerade Syndrome, or Purulent Component

Not all chalazia are alike. Certain presentations — including liquid chalazia, cases where masquerade syndrome is a concern, suspected virally induced lesions, atypical chalazia, and those with a purulent component — require a distinct clinical approach compared to a straightforward meibomian cyst.

Clinical scenario This protocol applies to liquid chalazia; cases of masquerade syndrome; virally induced chalazia (suspected viral etiology); atypical chalazia; and those that have a purulent component. Correct identification of the presentation type directly guides the management choice.
Treatment approach (partial) These subtypes are more amenable to a surgical intervention — incision and curettage (I&C) — than to injection-based management. When a viral etiology is suspected, a specific category of local injection therapy is contraindicated. Full protocol detail, sequencing, and decision criteria available via the link below.
