First-Line Treatment of Cervical Adenocarcinoma In Situ

Cervical adenocarcinoma in situ (AIS) is a preinvasive glandular lesion of the cervix. Definitive management is guided by the findings from diagnostic excisional procedures, and the protocol specifies a primary surgical approach along with criteria for additional intraoperative assessment.

This protocol applies to patients with a confirmed diagnosis of cervical adenocarcinoma in situ. Key considerations include the results of prior excisional workup and whether further surgical evaluation at the time of definitive intervention is warranted.

Treatment Approach

The recommended first-line management is a surgical procedure. The protocol additionally addresses the role of intraoperative staging assessment and the conditions under which it applies.

Full procedure details, decision criteria, and evidence grading are available in the complete protocol.

Simple hysterectomy is preferred for patients with confirmed diagnosis of AIS with negative margins on the conization specimen (BIII).

Surgical assessment of lymph nodes is acceptable at the time of hysterectomy (CIII).
