Treatment of Cerebral Meningioma When No Further Local Treatment Option Exists

Most patients with cerebral meningioma are managed with surgery and/or radiation. When those avenues have been exhausted and disease continues to progress, a different treatment strategy is required.

This protocol specifically addresses the clinical scenario in which no further local treatment option — surgical or radiotherapeutic — remains feasible, and a systemic approach must be evaluated.

In this setting, systemic salvage pharmacotherapy targeting angiogenic pathways may be considered. The full protocol defines which agents are appropriate, the conditions under which they apply, and what response to expect.

Pharmacotherapy using bevacizumab or multikinase inhibitors targeting VEGF receptors should only be considered if no further local treatment option exists.

The best albeit modest results with pharmacotherapy have been obtained with bevacizumab or multikinase inhibitors targeting vascular endothelial growth factor receptor, but no standard of care systemic treatment has been yet defined.
