Progression / Recurrence

Treatment of Central Nervous System Metastases at Progression or Recurrence

When CNS metastases progress or recur, management is determined on a case-by-case basis. Performance status, current neurological function, the specific nature of the CNS progression, and the patient's prior treatment history all shape which intervention is appropriate.

Clinical Situation

This protocol addresses CNS metastases that are progressing or have recurred. There is no single default path — each decision is individualised. The options and their sequencing depend on factors that must be assessed together, and the full algorithm requires evaluating each element of the patient's current status.

Treatment Approach

At progression or recurrence, options include local interventional strategies — such as surgery and/or radiation-based approaches — selected according to performance status, neurological function, and prior treatment. The specific combination and sequencing are determined by the individual case. The complete regimen, full list of options, and sequencing algorithm are available in the structured protocol.
