Treatment of Carcinoid Syndrome in Carcinoid Heart Disease with Right-Sided Valve Regurgitation

This protocol addresses carcinoid syndrome presenting in patients with carcinoid heart disease — characterised by right-sided heart valve regurgitation or stenosis, elevated urinary 5-HIAA, elevated NT-proBNP, and progression to right heart failure.

Clinical scenario: Carcinoid heart disease (CHD) is a rare and complex cardiac complication occurring in patients with advanced NETs and carcinoid syndrome, usually manifesting mainly as right-sided heart valve regurgitation or stenosis and eventually leading to right heart failure (RHF). Elevated u5-HIAA and elevated NT-proBNP are characteristic biochemical markers of this presentation.
Management approach

Management in this setting targets two simultaneous priorities: aggressive reduction of tumour-related hormonal secretion, and active treatment of right heart failure when present. Specific measures directed at symptomatic right heart failure are a core component of the approach — but they carry an important caution regarding how they are applied.
