Treatment of Capillary Leak Syndrome with Hypotension During ELZONRIS Therapy
This protocol addresses capillary leak syndrome (CLS) arising in patients receiving ELZONRIS, specifically when it presents as oedema (including pulmonary oedema), fluid overload, and/or hypotension or haemodynamic instability.
Clinical Scenario
Patient on ELZONRIS develops new onset or worsening oedema (including pulmonary oedema), fluid overload, and/or hypotension or haemodynamic instability — findings consistent with capillary leak syndrome requiring prompt intervention.
Treatment Approach
Management combines a corticosteroid-based regimen with aggressive supportive measures targeting fluid status and haemodynamic stability. The specific agents, sequence, and clinical decision points are detailed in the full structured protocol.
Treatment Goal
Resolution of capillary leak syndrome signs and symptoms — including oedema and hypotension.
References
- Oedema, fluid overload and/or hypotension
- New onset or worsening oedema, including pulmonary oedema
- Hypotension or haemodynamic instability
- Administer 1 mg/kg of methylprednisolone (or an equivalent) per day, until resolution of CLS sign/symptom or as indicated clinically.
- Aggressive management of fluid status and hypotension if present, which could include intravenous fluids and/or diuretics or other blood pressure management, until resolution of CLS sign/symptom or as clinically indicated.
