Treatment of Bronchogenic Cyst Located Within the Lung Parenchyma (Intrapulmonary)

This protocol addresses bronchogenic cyst arising within the lung parenchyma itself — the intrapulmonary variant. This anatomical location distinguishes it from mediastinal bronchogenic cysts and shapes the surgical decision-making.

Surgical resection is the treatment of choice for intrapulmonary bronchogenic cyst. The degree of lung tissue removed depends on the cyst's characteristics and location within the parenchyma — and the surgical approach has continued to evolve with advances in operative technique.

  1. In the case of intrapulmonary bronchogenic cysts within lung parenchyma, lobectomy has been the historical gold standard though more recently parenchymal sparing approaches, such as non-anatomic wedge resection or segmentectomy, have been advocated by some surgeons.
  2. With the emergence of minimally invasive platforms, resection of bronchogenic cysts either mediastinal or intraparenchymal variants can frequently be performed via thoracoscopic approach.
