This protocol addresses the specific clinical scenario of branch retinal vein occlusion (BRVO) in which the macula is non-perfused, peripheral retinal ischemia is present, and macular edema has developed — a combination that shapes both the assessment priorities and the treatment approach.
The macula is not perfused and the peripheral retina is also affected by ischemia. Concurrent macular edema must be assessed and addressed. The extent of peripheral ischemia is a key determinant of management.
Management involves intravitreal pharmacotherapy targeting macular edema, with the possibility of laser intervention directed at peripheral areas of nonperfusion in selected cases. The full structured regimen specifies when and how each approach applies.
Complete treatment criteria and sequencing are available in the full protocol.

On the other hand, if macula is not perfused and periphery suffers from ischemia as well, one should assess for macular edema and treat with either anti-VEGF or intravitreal corticosteroids, but laser treatment of the peripheral areas of nonperfusion can be considered if the area of periphery ischemia is very extensive.
