First-Line Treatment of Bone Metastases

When cancer spreads to bone, prompt initiation of bone-modifying therapy is recommended at the time of diagnosis — including in asymptomatic patients — and maintained throughout the course of disease to reduce the risk of skeletal-related events.

Clinical Situation

This protocol addresses first-line bone-protective management at the point of confirmed skeletal involvement, regardless of symptom status. Both intravenous and subcutaneous bone-modifying options are considered, alongside nutritional co-supplementation, initiated from diagnosis and continued on a long-term basis.

Treatment Approach

The regimen centres on bone-modifying agents — one delivered intravenously on a scheduled interval, and an alternative delivered subcutaneously — combined with supplemental support. Frequency of administration and the choice between agents depend on factors detailed in the full protocol.

The complete regimen — including agent selection, administration intervals, supplementation specifics, and continuation criteria — is available in the structured protocol below.
