This protocol covers patients with a gallbladder adenoma measuring 6–9 mm in whom at least one risk factor for malignancy is identified at initial diagnosis, and who are considered fit for surgery.
Gallbladder adenomas in the 6–9 mm range sit in an intermediate zone where the detection of any malignancy risk factor at the time of first diagnosis shifts the management pathway. Surgical fitness is a prerequisite before this approach is pursued.
In this setting, a surgical intervention is recommended. The decision to proceed is made after the patient is appropriately counselled and written consent is obtained. The full pathway — including procedural details and the complete decision algorithm — is available in the structured protocol below.

Cholecystectomy is recommended for patients who are fit for surgery if any risk factor for malignancy is found at the initial diagnosis of adenomas that are 6-9 mm in size, and surgery should be performed after patients are reassured and consent is obtained.
