Vocal Cord Nodules: Surgical Management When Non-Surgical Voice Therapy Has Not Achieved Resolution

Clinical Scenario

This protocol addresses patients with vocal cord nodules — a benign laryngeal tumor — in whom the initial non-surgical approach has not led to the expected improvement. The clinical question is what the appropriate next step is when conservative management falls short.

Previous Treatment Did Not Achieve Its Goals

The first-line approach consists of non-surgical, voice therapy-based interventions: vocal hygiene advice (including changes to the working environment), reduction of voice abuse, voice re-training, and voice rest. Exacerbating factors — such as infection, allergy, and gastroesophageal acid reflux — are addressed with appropriate medical treatment. When symptoms are not severe or spontaneous improvement is anticipated, observation alone may be chosen.

This protocol is indicated when that approach has failed to achieve resolution of vocal cord nodules on endoscopic examination and improvement in levels of impairment.
