Treatment of Basal Cell Carcinoma in Locally Advanced Disease
Clinical Scenario
This protocol applies to patients with locally advanced basal cell carcinoma (laBCC) of the skin. Locally advanced BCC represents one end of the advanced BCC spectrum and typically involves disease that cannot be managed by standard excision alone — whether due to tumour extent, anatomical location, or the overall clinical situation.
Patient Population
Advanced BCC encompasses both locally advanced BCC (laBCC) and metastatic BCC (mBCC). This protocol addresses the locally advanced subset, where careful clinical assessment drives the choice of intervention.

Advanced BCC includes locally advanced BCC (laBCC) and metastatic BCC (mBCC).
Decision on the potential suitability, indication, and technique in locally advanced BCC shall be made in a multidisciplinary team.
Radiotherapy is a valid alternative to surgery and may be considered as a primary treatment in patients who are not candidates for surgery (e.g. locally advanced disease, comorbidities, or those who decline surgery) or in cases when curative surgery is not possible or could be disfiguring or burdened by poor aesthetic outcome, including BCCs located on the face (i.e. eyelid, nose, lip) or large lesions on the ear, forehead, or scalp.
