Balanoposthitis in Penile Fixed Drug Eruption Not Resolved by Initial Treatment

Fixed drug eruption is an uncommon but recognised cause of balanoposthitis — the penis is one of the more commonly affected sites. Precipitants include non-steroidal anti-inflammatories, paracetamol, and antibiotics. When first-line measures fail to achieve full resolution, a structured escalation protocol applies.

Clinical Scenario

Patient presents with balanoposthitis attributable to fixed drug eruption of the penis, in whom initial drug withdrawal and topical therapy have not resulted in resolution of lesions.

First-Line Approach & Failure Condition

Initial management consisted of discontinuation of the precipitating drug combined with topical steroids of mild to moderate strength applied twice daily. The treatment goal — complete resolution of penile fixed drug eruption lesions — was not achieved, indicating escalation to the next protocol step.

Next Treatment Step (Partial Overview)

For cases with severe or persistent lesions unresponsive to topical therapy, a systemic intervention may be indicated. Full regimen details, eligibility criteria, and sequencing are available in the structured protocol.
