Bacillary Angiomatosis: What to Do When Initial Antibiotic Therapy Has Not Achieved an Adequate Serological Response

This protocol is for patients with Bacillary angiomatosis who have undergone a first-line antibiotic course but have not met the expected serological response threshold — requiring a structured next-line approach.

Prior treatment & failure condition

The initial regimen consisted of doxycycline or erythromycin as preferred options, or azithromycin or clarithromycin as alternatives, each continued for at least 3 months. Failure is defined as the absence of a fourfold or greater decrease in anti-Bartonella IFA IgG antibody titers on sequential endpoint titration, assessed every 6 to 8 weeks during treatment.
