Treatment of Mild to Moderate Babesiosis in an Immunocompetent Patient Not Requiring Hospital Admission

This protocol covers immunocompetent patients presenting with mild to moderate babesiosis. Babesia parasitemia is below 4% and the overall clinical picture does not require hospital admission. These patients typically experience mild to moderate symptoms and are suitable for outpatient treatment.

Guidelines strongly recommend treating this population with an oral two-drug antibiotic combination. A preferred regimen exists, and a distinct alternative combination is available. Complete regimen details, including drug selection, dosing, and duration, are available in the full structured protocol.

Resolution of symptoms, with fever and Babesia parasites on blood smear expected to clear within approximately one week of initiating treatment.

  • These patients usually are immunocompetent, experience mild to moderate symptoms, have a parasitemia <4%, and do not require hospital admission.
  • We recommend treating babesiosis with the combination of atovaquone plus azithromycin or the combination of clindamycin plus quinine (strong recommendation, moderate-quality evidence).
  • In otherwise healthy individuals, symptoms of babesiosis generally abate within a few days after initiation of treatment.
  • Fever and parasites on blood smear usually clear within a week.
