Treatment of Mild to Moderate Babesiosis in an Immunocompetent Patient Not Requiring Hospital Admission
Clinical Scenario
This protocol covers immunocompetent patients presenting with mild to moderate babesiosis. Babesia parasitemia is below 4% and the overall clinical picture does not require hospital admission. These patients typically experience mild to moderate symptoms and are suitable for outpatient treatment.

Treatment Goals
Resolution of symptoms, with fever and Babesia parasites on blood smear expected to clear within approximately one week of initiating treatment.

- These patients usually are immunocompetent, experience mild to moderate symptoms, have a parasitemia <4%, and do not require hospital admission.
- We recommend treating babesiosis with the combination of atovaquone plus azithromycin or the combination of clindamycin plus quinine (strong recommendation, moderate-quality evidence).
- In otherwise healthy individuals, symptoms of babesiosis generally abate within a few days after initiation of treatment.
- Fever and parasites on blood smear usually clear within a week.
