علاج ساركوما الليفية الموضعية — غير عالية الدرجة (G1 أو GX)
السيناريو السريري
يتناول هذا البروتوكول المرضى المصابين بـساركوما الليفية الموضعية بـدرجة نسيجية غير عالية — تحديداً الدرجة G1 (متمايزة جيداً) أو GX (الدرجة غير قابلة للتقييم). الجراحة هي معيار الرعاية المعتمد لساركوما الأنسجة الرخوة الموضعية في هذا السياق.
نهج العلاج
يرتكز التدبير العلاجي على إجراء جراحي، تُحدَّد التقنية المحددة فيه وفقاً للتوضع التشريحي للورم وخصائصه. تُوضَّح الاستراتيجية الجراحية الكاملة — بما في ذلك النهج الموصى به ومتطلبات الهوامش — في النظام العلاجي المنظم.
الهدف الأساسي
هوامش استئصال خالية من الورم نسيجياً (R0)
يُعدّ تحقيق استئصال R0 الهدف الجراحي المحدد للحدّ من خطر الانتكاس الموضعي.

- Surgery represents the standard therapy of localized soft tissue sarcomas.
- The surgical procedure depends on the tumour localization, its size and grade of malignancy.
- In case of intramuscular localized soft tissue tumours, the affected muscle compartment should be resected en-bloc as part of the so-called compartment resection.
- If on the other hand the tumours do not reach the muscle origin, and its insertion, or in case of an extracompartmental growth, a wide resection should be performed.
- If possible an R0, tumour margin-free, resection should be achieved in order to minimize the risk of local recurrences.
- Even though a 2 cm margin is sometimes recommended, a generally valid safety margin has not yet been determined.
