يتناول هذا البروتوكول داء الليشمانيا الجلدي الناجم عن Leishmania mexicana لدى المرضى الذين لديهم أكثر من ثلاث آفات يتجاوز قطرها 30 ملم، أو آفات في موضع تشريحي حساس، أو مرض لم يستجب للعلاج الموضعي.
اعتمدت خطوة العلاج السابقة على أساليب موضعية — العلاج بالتجميد مع ارتشاح بمركبات الأنتيمونيال، أو مرهم يستند إلى باروموميسين أو كلوريد ميثيل بنزيثونيوم — لمرضى لديهم آفات أقل عدداً أو أصغر حجماً. حين لا يحقق ذلك العلاج الموضعي إعادة التظهر الظهاري الكاملة (الشفاء التام) للآفة الجلدية، يصبح هذا البروتوكول الجهازي هو الخطوة التالية المشار إليها.

Treatment of L. mexicana
More than three lesions with diameter >30 mm, delicate location and/or refractory to topical treatment — systemic treatment:
Cutaneous lesions usually heal within a month after starting treatment with pentavalent antimonials, either by local infiltration [Old World cutaneous leishmaniasis (OWCL)] or given systemically [New World cutaneous leishmaniasis (NWCL)], but large ulcers may take longer.
Treatment failure is present when reepithelialization is incomplete 3 months after starting therapy.
