Treatment of Aortoiliac Embolism in Acute Limb Ischaemia — Rutherford Grade I (Viable Limb) with Acute Onset Claudication

This protocol covers the management of aortoiliac embolism presenting as Rutherford grade I acute limb ischaemia: a viable limb with acute onset claudication that does not threaten limb survival.

The patient presents with acute onset claudication in the setting of aortoiliac embolism. Rutherford grade I classification indicates that the limb remains viable and ischaemia is not immediately limb-threatening — a distinction that directly determines the appropriate management pathway.

Current evidence does not support interventional therapy for this Rutherford grade I presentation. A conservative strategy is recommended — the complete protocol specifies the components of medical management and the structured non-pharmacological programme involved.

  • For patients with acute onset claudication (Rutherford grade I) that does not threaten the limb, (percutaneous) catheter-directed thrombolysis is not recommended.
  • Therefore, patients presenting with acute onset claudication should be treated conservatively by best medical treatment and supervised walking therapy.
