Treatment of Antiphospholipid Syndrome in High-Risk Antiphospholipid Antibody Positivity Without Clinical APS

This protocol covers the management of patients who test positive for antiphospholipid antibodies with a high-risk aPL profile, but who do not yet meet the classification criteria for antiphospholipid syndrome — with no vascular or obstetric APS events on record.

Clinical scenario

Asymptomatic antiphospholipid antibody positivity with a high-risk aPL profile, in the absence of established antiphospholipid syndrome. Patients may or may not carry additional traditional risk factors alongside their aPL positivity.

Recommended approach

In asymptomatic aPL carriers (not fulfilling any vascular or obstetric APS classification criteria) with a high-risk aPL profile with or without traditional risk factors, prophylactic treatment with low-dose aspirin (LDA) (75–100 mg daily) is recommended.
