Clinical Scenario

This protocol addresses anogenital warts occurring in a pregnant person. Pregnancy significantly narrows the range of safe treatment options and introduces additional clinical considerations around delivery planning.

Treatment Approach

Removal of warts may be considered using provider-administered ablative methods. A number of agents commonly used outside of pregnancy are contraindicated in this population and must not be used. In certain circumstances, delivery route may also require clinical consideration.

Goal

Resolution of anogenital warts. Complete resolution may be more difficult to achieve during pregnancy; the full protocol addresses this clinical context.

Podofilox, podophyllin, and sinecatechins should not be used during pregnancy.
Imiquimod appears to pose low risk but should be avoided until more data are available.
Although removal of warts during pregnancy can be considered, resolution might be incomplete or poor until pregnancy is complete.
Cesarean delivery is indicated for women with anogenital warts if the pelvic outlet is obstructed or if vaginal delivery would result in excessive bleeding.
For most patients, treatment results in resolution of the warts.
