First-Line Treatment for Ankle Osteoarthritis

Ankle osteoarthritis is a chronic degenerative joint condition. First-line management follows a structured, evidence-based approach aimed at reducing pain, improving function, and supporting long-term joint health.

The first-line protocol applies to patients presenting with ankle OA who have not yet undergone a structured management programme. The approach is non-pharmacological, patient-centred, and built around education, activity, and self-directed care.

The first-line strategy begins with education and structured self-management — equipping patients to understand their condition and apply joint protection principles. Physical activity plays a central role in this regimen. Additional components address other modifiable factors.   • Education and self-management constitute a key component of OA management. Referral to self-management programs should be provided, and patients should receive education on OA, management approaches, joint protection strategies, and exercise.
  • Patients with OA should be advised to engage in regular low impact aerobic exercise (land or aquatic-based) and to lose weight if overweight.
  • Tai chi is recommended for patients with knee, hip, and/or hand OA, with strongest evidence in knee OA.
  • Walking aids and other assistive devices are recommended on an as needed basis to improve mobility and activities of daily living in patients with OA.
