Male AGA (Hamilton-Norwood IIv–V): what to do when Dutasteride failed to stop hair loss and induce regrowth

This protocol is for male patients aged 18 or older with mild to moderate androgenetic alopecia (Hamilton-Norwood stages IIv through V) whose prior treatment with Dutasteride did not achieve the required clinical goals, making escalation to the next therapeutic line necessary.

Clinical scenario
Male sex · Age 18 years or older · Mild to moderate androgenetic alopecia, classified Hamilton-Norwood IIv–V.
Previous treatment — documented failure
Prior therapy: Oral Dutasteride
Goals not achieved: cessation of ongoing hair loss and induction of measurable hair regrowth (increased hair count). Failure to reach these targets on Dutasteride is the escalation criterion that triggers this next-line protocol.
Next-line approach (overview)
The next step involves a surgical intervention — a form of follicular unit transplantation — considered when the patient has sufficient donor hair. This surgical approach is combined with an oral medical therapy to optimise the clinical result.
