Anal Fissure with Low (Hypotensive) Anal Sphincter Pressure

This protocol addresses anal fissures occurring in the setting of low — or hypotensive — anal sphincter pressure. Such fissures may be anteriorly located or present as multiple lesions, a pattern that distinguishes this group from the more common high-pressure presentation.

Clinical scenario
Anal sphincter pressures below the clinically and manometrically accepted normal range define this subtype. Because sphincter hypertension is absent, the therapeutic approach differs substantially from standard fissure management. The anterior location and potential multiplicity of fissures are further features that guide treatment selection.
Treatment approach
Management is conservative and sphincter-preserving — centred on lifestyle modification, topical analgesia, and stool softeners, alongside pelvic floor physiotherapy and biofeedback.
