What to Do When Botulinum Toxin Fails to Heal an Anal Fissure — or the Fissure Recurs

This protocol applies when a first injection of botulinum toxin for anal fissure has not produced healing, or when the fissure initially healed but subsequently recurred. It defines the structured next step after that first-line attempt.

Previous Treatment & Failure Condition

The preceding line used botulinum toxin at the lowest dose as a single injection. The expected outcome was healing of the fissure, with a two-in-three healing rate anticipated. This next-line protocol is indicated when that outcome was not reached — either because the fissure failed to heal, or because it healed and then recurred.

Next-Line Approach (Partial Overview)

A subsequent dose of botulinum toxin is considered in this setting. The full protocol specifies when and how this repeat injection is structured, and what distinguishes it from the initial course. The clinical goal is healing of the fissure.
