Treatment of Unresectable Injectable Melanoma When Systemic Therapy Is Not Suitable

In some patients with unresectable amelanocytic melanoma, standard systemic treatment is either not medically appropriate or not the patient's preference. When lesions remain accessible for direct injection — whether cutaneous, subcutaneous, or nodal — a distinct therapeutic approach applies.

Patients with injectable unresectable melanoma lesions (cutaneous, subcutaneous, or nodal) who are not eligible for, or not desiring, recommended systemic therapies. In this setting, a locally directed primary therapy may be offered.

  • For patients with injectable (cutaneous or subcutaneous or nodal) unresectable lesions who are not eligible or do not desire the recommended systemic therapies, talimogene laherparepvec may be offered as primary therapy (Type: Evidence based, benefits outweigh harms; Evidence quality: Low; Strength of recommendation: Weak).
