AIDS-related Kaposi Sarcoma Progressing on First-Line Chemotherapy: What Comes Next?

This protocol applies to patients with AIDS-related Kaposi sarcoma (KS) whose disease has not responded adequately to first-line systemic treatment given alongside antiretroviral therapy (ART). When lesions continue to progress despite that initial approach, escalation to a defined next step is required.

First-Line Treatment — Failure Condition

First-line systemic chemotherapy — including anthracycline-based regimens — was administered in combination with ART. The treatment goal was regression of Kaposi sarcoma lesions: a measurable reduction in the size, thickness, and coloration of tumours, along with resolution of edema. When that goal is not achieved and KS progresses on this line, the treatment below represents the next indicated step.

Second-Line Approach (Partial Overview)

The second-line strategy involves systemic chemotherapy — a taxane-based agent — continued alongside ongoing ART. The complete regimen, including drug selection, alternative agents, and monitoring criteria, is available in the full structured protocol.

Treatment Goals

Regression of Kaposi sarcoma lesions — assessed by reduction in the size, thickness, and coloration of tumours, and resolution of edema.
