Treatment of Adult T-cell Leukemia-Lymphoma in Smouldering, Primary Cutaneous, or Chronic Subtypes

Clinical Scenario

This protocol addresses adult T-cell leukemia-lymphoma (ATLL) in patients presenting with the smouldering, primary cutaneous, or chronic subtype — presentations that are distinct from the acute and lymphoma subtypes and require a specific first-line approach.

Which Patients

Patients with smouldering ATLL — including those with skin or lung lesions, or opportunistic infections — as well as those with primary cutaneous ATLL or chronic ATLL are the target population for this regimen. All three presentations are considered appropriate for this first-line strategy.

All patients with 'smouldering' (skin/lung lesions, opportunistic infections), primary cutaneous, or chronic ATLL, should be offered AZT/IFN-a treatment (GRADE 1C).

Lower dosing is also used in chronic/smouldering ATL or when given concurrently with chemotherapy in the lymphoma subtype.
