Treatment of Lymphoma-Type or Bulky Acute Adult T-Cell Leukemia-Lymphoma

Adult T-cell leukemia-lymphoma (ATLL) presenting as the lymphoma subtype, or as acute ATLL with bulky disease, requires a chemotherapy-based approach. This protocol defines first-line management for that specific clinical scenario.

Clinical scenario

This protocol covers patients with lymphoma-type ATLL, and patients with acute ATLL who present with bulky disease. Nodal and lymphadenopathic involvement is characteristic of these presentations and shapes the treatment strategy.

Treatment approach (overview)

A CHOP-like chemotherapy regimen forms the backbone of management. The complete protocol specifies additional interventions, eligibility criteria for consolidation, and the full structured algorithm — all of which remain beyond this summary.

  1. Patients with lymphoma, or bulky acute ATLL, should be offered CHOP-like chemotherapy and consideration should be given to concurrent AZT/IFN-a (GRADE1C).
  2. Nodal disease (lymphoma or acute subtypes with lymphadenopathy) should be treated with a chemotherapy-based approach.
  3. CHOP-like protocols remain standard of care.
